Not enough information to determine what the cause is. Could be a foundation issue, could just be an old house. Not all uneven floors are a cause of concern.

Depends on what is underneath. If it's floor joists it's a pretty easy fix. If it's concrete, it depends on cause. A shitty finisher can have low spots and you just float a self leveler. If it is structural failure you should learn to live with it or bring some cash to jack that shit back up and fill underneath.
