OP, I am a resident in general surgery that is interested in going into colorectal surgery, so I know a bit about this area of the body.

What you have is something called pilonidal disease, although you have a somewhat unusual presentation of it. Typically with pilonidal disease, you will get small pits (little holes in the natal cleft that begin are caused by skin pores enlarging) and these pits will get hair and debris stuck in them, which causes a cyst/abscess. Usually that cyst will be at the side of the natal cleft and result in a painful lump that has to be drained. Sometimes a sinus tract will develop, too, which will drain pus and fluid consistently. I have attached an image of what normal pits and a cyst looks like (the cyst will be much bigger when infected).

What you have is a pit that has enlarged significantly to the point where it can be considered an open wound. Likely you started out with a small pit, hair from the buttocks and back of the head lodged in it, and it has dilated (enlarged). The fact that there is pus draining from it too implies that there is likely an abscess underneath it. It's also in close proximity to your anus, too (normally pilonidal disease is higher up in the cleft), so that complicates things a bit.

I would recommend that you go and see a pilonidal specialist who specializes in a procedure called the cleft lift. The ordinary procedures that general surgeons perform for this (closed and open excisions) likely won't work due to the complexity of your disease.

He's not going to die from pilonidal disease. It can be painful and make day-to-day life miserable, but it won't kill you. It's essentially a disease of the skin. He will almost certainly need surgery to fix this, though.
