I'd imagine I'd probably scream briefly if I saw a stranger explode too. For you to fall out two different mechanisms have to fail, the shoulder harness which you can see in the video still in place and not fail, and the seat belt that keeps people from squeezing between the crotch and shoulder harness...it's latch is easy to unbuckle. If you watch the video you can see him already out of the seat during the fall, which can't just accidentally happen

Honestly pretty sad, but I suspect this was retardation on his part, although the shoulder harness probably did strap in, and the seatbelt was there, it is very easy to unbuckle and slide underneath, after all they aren't made to keep locked in who want to get out
