I hope you're OK and I know its easy for me to write this and a lot harder for you to do it but this was my steps out of depression.
1. Write down why you are depressed (for me, there was 3 reasons, job progression, unfit and bad GF),
2. Write what you are going to do about it.
3. Write how you are going to do it (writing a schedule helps, but I dumped my GF as that was an immediate change, I did some independant IT qualifications as that was the field I was working in and started to go to the gym),
