Pretty close, but I'd likely be dealing with refugees more than anything.

This. It assumes a 500 warhead scenario would be counter-value/population and as such the triangles are all over airports and cities. Some of which would no longer be worth a warhead in 2022. No one is nuking Binghamton NY. The only thing left there is vacant buildings and heroin.

2000 warhead scenario appears to target military bases, ports, nuclear power facilities and missile sites. The sea of black dots are the Minuteman launch fields in the midwest.

A counter-force attack wouldn't necessary come with a counter-value attack but a counter-value attack would generally always be preceded by a counter-force attack.
