I think it's probably very similar to autism. You have to learn your empathy cognitively. You can't feel with people, so you have to logically extract what they are feeling. is right to some extent, you are pretty cold in general. You feel detatched, because you can see how nomselfish and impulsive many people act and you can't emphasize with it. Also, lack of a Conscience makes everything easier, but probably a bit muted.
In general, I feel like I'm more logical tha other people, more selfish and more controlled, but I think Ibmiss out on a lot of the deeper social aspects of life. I hunt either for validation (because that's the one thing I do feel) or immerse myself in logical work like maths or CS, because I can't get the full spectrum of emotion from human interactions.

And that reason is in most cases lower inhibition, lack of a conscience and lack of empathy. The reason sadism often concurrently shows is that this type is often trauma/childhood-related and an abused child often also develops sadism.
