There have been a couple FE games with incest going on. Most notable example is in FE4, but Fates also had it with pic which is hilarious because the game made an extra effort to avoid the obvious incestual implications of Corrin hooking up with any of the royal siblings but for some reason pushed Azura who is Corrin's cousin.

But iirc there was something after the release of Fates that retconned Azura being Corrin's cousin, but I don't think that was in-game material so it may not count.
