Stealth works for me?? You can also enable the experimental stealth feature if you want extra stealth, but default is okay for me.
If you use a silencer you can get away with a lot.

Are you supposed to start on easy and ramp up with better gear? I started on medium and took a starter quest to kill some boars, then got swarmed by eight of them and was two-shotted along with the NPC who was escorting me. Higher difficulties in the base games were challenging but fair
