Just like almost all other open world games, it relies way too heavily on repeated content. It's not the worst offender, and the fact that you get unique rewards for everything you do is better than most other open world games though.

She does show up at other graces.
The issue is that for some reason they decided that you need to choose a "Talk to Melina" option at those that you probably completely ignored by accident because it only shows up at like ten out of the threehundred gorillion ones and she doesn't appear at those graces otherwise to make it more clear, be it already standing there or teleporting in like at the earlier ones.
