You're not supposed to 'aim straight', there's no barrel. It's not a literal gun, pulling the trigger simply produces vibration on the blade that's supposed to act like the high frequency swords from Metal Gear and help cut shit better. It's why in-game you pull the trigger during an regular slash attack

it doesnt' shoot bullets, the rounds you put into the revolver don't have a jacketed bullet, but simply cause a resonance from firing the round.

basically it vibrates the blade at a specific frequency to make it cut better.

Japanese do not into guns. Also gunblade doesnt actually shoot anything pulling the trigger just causes the blade to vibrate for extra damage or something like that.
