OP pic is suffering from serious heart problems, the bloated toe is from excess water retention. Nail itself is being eaten by a fungus, which causes discoloration and thickness of the nail.

you don't need a doctor but you definitely need OTC anti-fungal stuff. and while you get that, buy some gold bold and sprinkle it in your shoes twice a day.

nah. it's far more likely the appearance of the toe is entirely because of the fungal infection.
