From the moment you get to the tutorial? Controls are janky, specifically the fucking disarming a LAM on a wall
From the moment you get to the main mission? AI fucks you over, if the enemy gets killed by your allies? It counts as you doing it and activates the trigger

Interestingly, there's no way to activate Tracer's hideout door without advancing the plot but you can actually just go straight to the elevator in Versalife
