There is no explanation. The game's lore is fucked to a frankly incompetent degree considering the scale of the production.

You can argue that From always had obtuse storytelling but when you came right down to it, Dark Souls lore made sense, mostly, maybe there's a few contradictories areas but nothing you can't smooth over or charitably excuse as diverging vision over nearly 10 years between the first and last game. Sekiro made sense too. Bloodborne don't seem to trouble people much. But ER is honestly a mess and anyone pretending otherwise is just confusing the truth (or lack of) with their own headcanon.

Rune of Death only enables the 'True Death' that is complete removal from the mortal world. Beings still can be killed in the lands between, but then their souls go back to the erd tree and can be reincarnated.
