Her teeth have little to do with it, it's the fact Mortal Kombat X gave her more of a brain. That attribute was more or less kept into 11 and her Tarkatan-esque teeth came back, which is fine.

Adding wax lips on top of the teeth is a massive part of it. It's the same thing that happened to Reptile: a huge fan favorite that the devs shit all over repeatedly by making his design worse and worse with each game. Mileena, at least, only suffered for one game but you never know when they'll fuck up again.

MK11 Mileena with MK9 Mileena's outfit would be a 10/10 Mileena design

Mileena always 'had more of a brain,' it's just that in MK9 she was just born and thus had zero experience. Like most characters in the series she was developed from MK2 all the way to Armageddon.
