How do you spend thousands of hours on this? I tried it out but it gets boring as hell after the first couple of runs

Alright OP, let me explain. It's for the same reason people play online games even if the objective is similar and doesn't change(ie battle royale or mmos or any multiplayer or rouguelite or cardbuilding, etc), because at the basic level they have the same objective but the way it's done is never the same.

There are a lot of tricks to get better items and better runs. The game really pops off once you get a couple good items because the big selling point of the game is the item synergies. The many items can combine in interesting ways sometimes.
It literally gets better when you unlock more shit. It becomes a game about resource management and item manipulation. You get to a point where you can manipulate almost any run into being broken if you know how to use cards/item spawning items/rerolls correctly
yes, you're missing several types of mental issues to be able to delude yourself it is a good game.

Download the steam workshop mod that tells you what items are and how they effect your stats. Makes the game actually playable
and then you unlock T R A S H items and the pool of items goes to shit.
If im honest, this game is at its best when you are mid way through it, on the late game is become lame as hell
late game i mean after unlocking 70% ish of items

I have everything unlocked and I can manipulate about 80% of runs in my favor. Use the fucking tools you have, be smart, do Devil Deals because Angel Deals are for non-aggressive safe-playing faggots

yes i love playing aggressive and getting shit like any of the 2389 babies that flood the pool, a mediocre at best active that isn't worth its price or something that literally hinders your run

Yes you can always make a run work, but don't pretend the game didn't fuck up the item pool and bloat it with a stupid amount of items that range from useless to hurtful
Game would be improved a lot if they added like a dozen straight boring DPS upgrade items among all the pools rather than zany shit that doesn't give you more damage item #314536152
False ultra greed has the gay damage reduction so that means it takes at least 3 minutes to kill

I just bought the game during the summer sale and it's fun but I'm really confused why some of the items you pick up don't even tell you what they do, like the cards and held items.
Well yes if you only got good items the game would easily turn into a cake walk after 10 unlocks
It just doesn't apply to Repentance. That's an Afterbirth+ complaint. Repentance has reverse cards including Judgement? and The Stars? and The Fool?, Magic Skin, the Spindown Dice, an incredible secret room pool, Eternal D6 for The Lost, Tainted Lost
If you're not able to get good items with all these tools, that's on you.
just make the dogshit items give you same usefull stat like DMG or tiers up or you know FUCKING SOMETHING
Nope, he took literally 2 seconds to kill. It was hilarious because I didn't even need to complete the run. Had green brimstone beams spawning from nowhere, going in random directions, fucking spinning in circles, and lingering long after the other tears were gone.
if i wanted to play a ultrahard run everytime i play a run, i would just do the challenge again and again
You aren't missing anything. Just play a better twin stick shooter like Robotron or Smash TV.

People might call it spoon feeding but the mod that makes it so there's a little explainer text on the screen for every item/trinket/consumable you see is a must have for me
