They do explain though. He regrew the eyeball multiple times but because demoman was cursed his eyeball grows into a a 3 or so meter sized floating eyeballs that shoots(I think) rockets every time and then they have to kill it.

Medic actually did regrow Demoman's eye, but Demoman was blackout drunk at the time and doesn't remember. Demoman still wears the patch because he honestly doesn't know life without it at this point. He also takes a lot of pride in being a "good demoman" because he is still alive and hasn't blown himself up yet, but still plays the victim for attention by constantly bringing up the eyeball thing.
Medic, understandably, keeps his damn mouth shut, because at this point it's been way too fuckin' long since the healing, and being questioned by an absolutely irate drunken Demoman would get in the way of more important science.
