Hitmen are allowed to eliminate anyone that a corporation desires, some for being a threat to profit or just for fun. Body horror is also a casual thing.

Worst Non vidya setting I can think of is the Plain of Stone Faces. But I'll mention the R type universe here since other settings have been brought up. Imagine fighting against what amounts to the concept of evil that can infect you by just thinking about it - and you wouldn't even know you've been infected by it.
