Pretty sure STALKER worked like that, the harder it was the more your aim mattered.
Easy mode, you can spray and pray back and forth with the enemies, nobody's aim matters.
Veteran, if you don't shoot them in the head then they'll shoot you in the head, and you will die.

Didn't Metro 2033 have a really good hardest difficulty like this that you only got from pre-ordering? And people said that it felt like the best difficulty of them all. I might be wrong, I own the game but never bothered playing it

In SoC and CS enemies will still usually just flinch from a headshot before dying at the second one, in CoP the flinching animation is very rare in comparison.
On an unrelated note I still find it weird that enemies can get bleeding and bleed to death but it's very rare for it to happen in both games.

Hard Mode in DD is both easier and harder than Normal. It's easier because every mob drops 10k+ money bags and you become overleveled from the XP modifier. But then you get to BBI where some enemies like the Fire Drake can one-shot you regardless of your gear.

Persona 5 Royal actually made the "hardest" difficulty easy because they tried too hard to compensate for the fact that you do .65x less damage but take 1.6x more damage. In exchange you get 1.2x exp and money and weakness and technical damage you deal is TRIPLED. You can essentially easily one turn every normal encounter because of this. Bosses typically don't have weaknesses besides two of them though, so bosses are harder. Funny enough though, one boss' entire gimmick is abusing elements so people actually suggest to turn the difficulty up to Merciless because it makes him easier. For the most part, hardest way to play P5R is to play on hard for normal fights and then turn to merciless for bosses.

Played a stealth action game called Intravenous that did this. It was harder than "True" difficulty because you only got one save per mission, but the shooting was easier because the enemies died in one hit

This is why I like games like stalker where your enemies have the same health as you.
Ranger Hardcore (I think) would have you instantly die by a headshot and same with the enemy. It also disabled the HUD and made ammo more scarce.
The original with no HUD was kino.
STALKER difficulty has nothing to do with the argument of the OP.
