In every run, I end up with Axe and it's upgrade. I can't help it, it's too good. Also at this point I always pick Mad Groove at the start, it makes every stage library 2.0.

Both.

A little bit. You can get more cards which give you different terrain, which can effect battles differently.
Really, the core of the loop hero "run" in managing where your terrain is placed and how it might change when other tiles are put in contact with it. (For example. Putting a forest next to a desert turns it into a marquis. Which gives different bonuses)
