I was 8 when this came out and it was and still is a really good game. Soundtrack is awesome (ost and radio)
Police system was fresh and new for its time. You can get a ticket for speeding for example. Gunplay and driving is a lot better than GTA 3 or Vice City. Also the story is somewhat similar to goodfellas.
Its not a sandbox game though, it is a story based game with an open world design.

Wonderful atmosphere, great story, an open world that serves as a backdrop for the plot rather than gameplay filler. It's certainly rough around the edges, with some of the gameplay being straight up bad. Enemy AI can be annoying, car chases can feel awkward, and the tommy gun acts more like a sniper rifle if you tap to shoot. But it's still a lot of fun to experience

For me the first mafia felt different than GTA 3. The storyline felt more realistic. GTA 3 felt bizarre and unrealistic. Mafia was more intricate and enjoyable. It was amazing for the time but I dont know if I would play it again.
