I honestly don't think that Kojima was thinking about that when he thought about the medic twist, he just wanted to make (You), the player, become Big Boss and continue some of the themes he talked about during MGS2
I don't recall any of the characters framing the body double plan like some kind of "bad" thing, so that makes me think that Kojima didn't realize what he was really doing

I took it that the reason none of the characters see it as bad is because they're all in the cult of Big Boss too and would do anything for him. It's up to the player to see from the outside that its a total supervising dictator move.

Because you know the truth, but what is said of his actions say otherwise: he had nukes on his base that threatened the whole world, he slaughtered innocent kids in a mine, he killed his own men... There's a difference between his image and your perception of it, that's where the demon lies, he had to pretend all those things just to keep people safe.

Enjoyed the game but the "players (hackers) can raid your base when you're offline and your singleplayer base WILL be effected by this" turned me off when I got to that part. Which was a shame.
