It heavily depends on the kind of game, but in general I'd lean towards not having traditional difficulty settings.
Instead there would be a single intended experience, plus
>a hidden achievement for completing the game while abstaining from certain features (self-imposed hard mode)
>an accessibility menu, with plenty of features to make the game as easy as you want- but I would make no attempt to balance the game around those features, and enabling them would lock out achievements

unironically I would like a "developer intended" difficulty indication since so many games the hard mode or harder feel like they should be the normal mode or different modes have actual changes to AI and stuff that isnt just different damage and health

Developers should either do away with using the easy/normal/hard difficulty names, or be better at making things actually hard.
Nowadays when I read "hard" I assume it will still be easy, because there's just so many games that have 1-3 difficulties above hard for no reason.
Hard doesn't fucking mean anything anymore. Games are never actually hard on hard.
Why even name it "hard" when you're gonna have extreme, super-extreme, SUPER-HARDCORE-EXTREME above it too?
