Always be in close quarters. Most of his deadly attacks are long range. Its far easier to react when you are in close proximity. Hesitation is defeat.

You have to hit him back. You will be at 50% poise the full fight, but it never breaks if you keep parrying. His poise jumps faster each phase, he literally gets tired, while you don't as a zombie. You just have to stop being afraid.

Hesitation is defeat. Keep attacking, parrying, dodge almost never after first phase, use the umbrella for the white attacks. Parry every spear move, you should be able to do it after 20-30 attempts learning the patterns.

OP, are you mashing or deliberately pressing buttons? Are you being aggressive? What prosthetic tools are you using?
It's unironically a way harder boss if you're scared of him. Attack(but don't mash, only press again if you see he didn't deflect your hit) so he can't start his combos, go on the defensive only after he deflects.
Second phase is definitely the hardest, which is when I recommend you use firecrackers to get free openings so you can get through it faster. Third phase can be easier due to him using lightning attacks that are easier to dodge. The spear swings are weird to dodge, but it's really the only tricky thing he has.
People who have a hard time with sekiro usually never realize just how powerful attacking is in this game. Force them into blocking until they deflect you, not only you do posture damage you also limit their options.
For Isshin specifically, his spear phase is the hardest. If you can't figure out his strings in this phase, the easiest way to get through it is to keep your distance and wait for the thrust at the end of his long combo that you should mikiri. You did master mikiri counter, didn't you, Op?
His last phase is a shortcut to victory thanks to the lightning reversal.

The one thing I love the most about Isshin is that he's an absolute bro throughout the entire game. Even when he fights you at the end there's just massive mutual respect all the way through. He might be my favorite boss of all time just by that alone and he isn't even a rival, he just likes a good fight.
This fight is so different making full use of the tools available. I beat him (and everything else) without using sugar, mortal draw, or mist raven (just pressure and deflections / dodges / counters etc); I did use the ichimonji art earlier in the fight, and the firecrackers on occasion. I had all of that stuff and wish I'd taken more time to play around with it so I could be better like your vid.
