probably losing control and not knowing whether something in the game is causing it or if the game just fucked up
i remember downloading a scary creature mod that when spawned it would seclude itself to dark places in the map - i spawned it into gm construct and it hid itself in that underground parkinglot, it would play ominous music when it was doing shit and you couldnt kill it. when i used the remove tool on it something either happened or must have been made to happen and all of a sudden some rising string stinger started playing and some parts of it were not removed. 14y old pants shitting fear ensued and i feverishly quit to mainmenu
