It transforms at the end of the turn. If you want to heal it run Rest, it's fat enough to where Rest-Sleep Talk-Coil/DDance-Thousand Arrows is more than enough to be immortal.

Why should the protector of Kalos be bigger than the planet? Rayquaza protects the ozone layer and it's relatively small as fuck. How would that even work when its physical mass is distributed through cells that are located on the planet itself?

First, it's not just the protector of Kalos, but of the entire Ecosystem.
Since Zygarde needs to be in either 10% or 50%, it can call out all the cells at once if necessary, and can adjust it's position during it so it's not destroying anything on the planet when growing to it's final size.
