Assuming you're not taking the piss, if Lance used to be the Champion before being beaten by Blue, I take it to mean Lance got demoted to Elite Four. Since they're the Elite FOUR, who was the fourth one if Lance was the Champion? Did Blue invent the title of Champion or something?

The game wasn't designed with the idea of the champion being a formal position, it's just the conclusion of the arc of your rival always being one step ahead of you that oh he also beat the elite four too so I guess the only way to determine the real champion here is for you two to fight.
Then much like how Obi Wan's incognito desert robes became the official jedi outfit, it just became standard for there to be a champion after the elite four, and for them to be someone you run across on your journey that you may or may not be told in advance is the champion.

No, he was simply the first to have it. The Elite 4 were there to test the future champion. This is like asking "who was the Dragon Warrior before Po?".

There was none, the Champion was whoever beat the E4 and that was Blue.

The real answer is that none of this was planned. There was never meant to be a gen 2 and the idea of the champion as a formal, permanent position didn't exist in gen 1.
As for the in-universe answer, I think it was something like this.
Lance was the champion, and Agatha, Bruno, Lorelei, and a mystery person were the E4. This mystery person might have been Will or Karen, but also might not have been.
Blue becomes champion, bumping all of them down one position and pushing the mystery person out of the E4. Blue gets beaten, Red fucks off to Mt. Silver, and Blue isn't interested in keeping the position so everyone gets bumped back up one position, to where they were before Blue came along.
Over the next 3 years, Lance stays the champion, Bruno stays the 2nd highest E4 member, Lorelei leaves (for Four Island business), Agatha leaves (or dies), and their positions are filled. The mystery person is either replaced or stops being a mystery.

No, the mystery bumped off E4 member doesn't work. Lance's dialogue makes it pretty clear he was always the final Elite Four member and the Champion spot was simply left vacant for some reason.

It's worth noting that Oak is allowed to enter the Hall of Fame, which according to future games means he beat the League himself once.
Based on that, and his background with Agatha, the games seem to imply Oak was in fact the previous champion and retired years or even decades prior, possibly a remnant of his planned boss fight.
As to why there was no active champion in the meantime, the best explanation I can think of is that there were no League worthy trainers for a long time, not even to fill the Elite 4 until Will and Karen arrived and Koga got promoted. So instead of promoting Lance, they prioritized keeping the E4 intact and left the official champion title vacant. This all makes sense when you consider the next logical choice for the E4, Giovanni, was largely absent.
