Trump had been musing about being president since before Obama and making offhand comments about the presidency. You can blame Sarah Palin for the ramp up in rhetoric. It was Palin and the McCain campaign that started calling Obama a muslim and terrorist. Trump ended up getting a bunch of press because of Fox News putting him on blast and twitter magnifying his reach. Without Fox and twitter, Trump wouldn't have been the loudest voice questioning Obama's citizenship. Also, Trump tended to say the quiet part out loud. He said out loud what Fox News implied or politely ignored.
