It's like any other ego-disorder. At the level of your core identity, not enough happened in the very early stages of development to make you "male" or "female" in the head. It's not anything that happened to you per se, it's things that didn't happen at a critical moment that you needed to experience in order to form a strong 'male identity' or 'female identity.' For example, having a male archetype around that you interact with or see interacting with.
Without that core male identity or female identity becoming internalized, it's very easy to be comforted by thinking you are the opposite. It's basically a psychotic delusion that is so deeply ingrained its coming from the core identity itself.
