Because I care about social cohesion more than even public health, I'd allow smoking but I'd penalize smokers by making them pay an extra tax that would cost exactly how much it costs the state to take care of them on average when x% of them get lung cancer.
So for example if they crunched the numbers and let's suppose it costs the state $5 billion a year to take care of everyone that has smoking related illness. Now it's say there are 40 million smokers. That means each smoker pays $125 tax a year to smoke and also have full access to healthcare as everyone else.
I'd do the exact same shit for heavy drinkers, being a fatass, repetitive auto accident causers.
So in this way even if you disagree with a smoker or a fatass, at least you know they are paying their fair share so would hate them less. Which means greater social cohesion.
This system would work extremely well as long as it was in a nation of all white people, since whites don't even need to be coerced to want to pay their actual fair share. They just get mad when it isn't fair.

what people don't like about smokers is that they stink, they cough too much, and their sense of taste and smell are fried. A pack per day seems okay to me, but there are lots of people that smoke 3 or 4 or 5 packs per day and there is nothing to be gained from that. Like okay maybe drinking a Coke every once in a while isn't terrible, but drinking 2 12 packs per day is entirely fucked.
