The primary reason for using drugs/surgery is to make the patient infertile with plausible deniability. This is the number one concern: fewer reproducing adults. The people at the tip top are using every method in their power to do this, to reduce human population, including all other facets of the rainbow coalition. Those doctors who go along with it are first intimidated to go along with it, and the ones who contribute directly are paid handsomely for their work. So the answer to your question is "all of the above and then some."

Although on the other hand, the way things are going it would be more likely used as an active influencer of the situation than a passive, harmless fantasy world.
In the end people will do what they want to do anyway, regardless. It's no ones business as long as they don't hurt others. That being said, I don't think transition surgeries should be taxpayer subsidized. Its unfair to everyone else, is questionable in morality (eg. "I'm paying the government to do WHAT?!!?") and gives the government incentive to ( what is in essence) sterilize it's own population. I mean... why even pay taxes.
