I think it's possible some deaths occur after the "active" rash phase of the infection and aren't attributed to monkeypox. If someone dies from encephalitis, hospital doctors probably won't associate the patient with monkeypox if there aren't active lesions at the time of death. This is all speculative of course. I agree the death rate so far doesn't seem to be keeping pace with the NTI predictions
