Probably a mixture of attention seeking and mental illness (self-inflicted and clownworld-caused) leading them to want to either want to "express themselves" (for attention) or change themselves in some way (they project their discomfort with themselves as being due to a bodily change being needed rather than them correcting a psychological or spiritual fault).
