Wait, how does this work in your countries, exactly? Because we also have free medicine here. I come to my local clinic and I get whatever services I need. If the clinic cannot provide these services (for example, I'm going to need a complicated surgery this week and they just don't do those), they send me to another institution, where the service is then provided (again - for free)..
... for that institution to send me BACK to the clinic would be absolutely retarded. It can happen, but it would mean that someone fucked up along the way. Royally. It's a legitimate reason to be very mad with someone.
we have mandatory! health insurance in goymany, cheapest you can get is just 250€ per month. if you dont pay you go to jail til you can cough up that money you owe them (+ interest). if you go to the Doc you have to pay up too or else you get the cheapest treatment of all.
Oh, we also have somewhat mandatory health insurance in Russia. It's free and if you don't get it - no free medicine for you.
You can still get paid services and... as is with any service under capitalism, the best one will be provided by those who you pay, but in my experience it basically comes down to "how polite they are".
A free therapist will be very curt with you while a paid one will be extremely polite. Both will usually prescribe the same thing, though. Paid one - probably the "same thing, but more expensive".
It’s pretty much the same in Japan, but there’s a health insurance from the government that allows you to pay only 30% for meds and surgeries etc (costs from 20$ to like a 1000$ (?), depending on your salary).
