The caveat is it’s dangerous to vax everyone with smallpox vaccine because you need a working immune system for it to work. The smallpox vaccine given throughout the 20th century was not an attenuated virus, but a live related virus (vaccinia, most closely related to horsepox). Makes sense it should not be given to immunocompromised individuals.
