The alternative American IMF debt leaves countries in ruin and you don't even get infrastructure, you just get American NGOs telling you to stagnate your country to drive down your wages for their benefit.
China wants mainland trade route with Europe to avoid US dominated seas that could easily block them from trading. Belt and road makes a long of sense for China but not so much for countries that take a bait to take huge loans only to have 100% chinese owned and built infrastructure on their soil. Most of projects are unprofitable, the one that bring profit is railway link to Kazachstan and then to Russia / Europe, there are actually a lot of railway cargo on that route.
