It is ethically questionable, but doctors do not take an oath to do no harm. Doctors do harm all the time, it's part of the job. Turning off life support, aborting babies. Even just exploratory surgery is doing harm. Heck, regular surgery hurts people. Prescribing medicine brings along side effects.
I imagine this doctor talked to his colleagues about it and signed all the forms saying that idubbbz was of sound mind, and the benefits to charity likely outweighed the dangers. Probably not the best for a doctor to be punching and getting punched, but it's a good cause.
