People with dementia don't fully understand all that is being said to them, and they often just cling to words or themes they recognize, without actually recognizing the context of what is being said, and then launch into responses that have nothing to do with the context based on these words or themes that they do understand. Biden does this practically everytime he speaks. The vacant stare is another indicator. People with dementia get this eyes glazed over look that Biden always has. Another giveaway is that you can see it getting progressively worse over time. It's not like his cogency has gotten better, it's gotten worse, as it will continue to do.
