It's no secret, people with the covid vaccine are given preference for transplants (as one of several factors).
When someone is on the list for a transplant, the doctors and pharmacists will go over their health records with a fine toothed comb, and scrutinize them during checkups and the like to gauge how likely they are to adhere to the anti-rejection meds, and lead a generally healthy enough lifestyle that they'll die in ten years instead of two.
Honestly at that point they have nothing to lose by taking the vaccine; the meds theyre on suppress their immune system for the rest of their life.
