Unironically homophobia and drug usage. When gays can’t just have stable relationships out of fear, it drives the sexuality to quick anonymous encounters. People get tested less often out of fear. Combined with intravenous drug usage, the virus will quickly spread under these conditions. Almost every homophobic country on earth has higher rates of AIDS than their liberal counterparts.

High levels of aids is usually present in low education areas , look at Africa , a lot of Russia is third world tier so I assume lack of education + unclean needles for drug use from poor addicts , i dont know how religious avarage russian family is but if they are i guess sex would be taboo and that would prevent younger people from getting tested ,a lot of russian women are pretty so I assume ateast some of them whore out to earn
