Its a lost art. Hedges used to be extremely common even serving as artificial borders.

Is there stone structure inside of it? Or is it purely made of plants? If only plants, they must be hundreds of years old at least, and only can survive in politically stable areas like parts of France, Italy, England. Surely a luxury if it's only plant

It looks cool but it fucks up walls over time. It dries out bricks and mortar causing it to crumble and then the ivy can root into the cracks causing more damage.

What about Asian gardening and horticulture? I think many temperate fruits originated there in prehistory along with quite a few ornamental plants

Ivy has surface binding roots, not burrowing roots. They do not grow into cracks. You can pull a 200y old ivy from the wall with one hand.

How does it dry it out? Dont they fetch water from the roots instead?

depends on the vegetation. this one should have a wall inside but it's possible it's a hedge with a strong base. they can be shaped in any form. some grow really fast as well.
