It's not dementia, he'd have more difficulty speaking. I've seen him speak and he doesn't seem to be suffering from any kind of significant cognition or motor function disorders.
His face has seemed more puffy as of late, but any number of medications can cause that. Assuming it's simple water retention, pills for managing cholesterol or blood pressure (both common prescriptions for someone of his age) include that as a fairly common side effect, especially if newly taken and the body is getting used to them.
Could be any number of other drugs, too, obviously.
