As considered in popular media? No, not even close. The US and Russia working together could not come close to directly killing everyone in the US alone. Little alone striking each other with a focus on strategic (away from population centers) targets and ignoring the rest of the world. So not even close.
But there are some things that would happen that most might not expect, for one it would be likely that the energy grid would be completely destroyed as it went down from multiple failures. Just as an example.

The thing to note with modern MIRV nuke clusters is it come in Heavy 800 kiloton 10 warheads load or Light 500 kiloton 15 warheads loads

More a matter of where it detonates. Ground detonation causes more radioactive fallout, but less damage than an airburst. Underwater causes lots of radioactive fallout as well.

High yield single warheads aren't as effective as multiple smaller warheads, as buildings would shield each other, what multiple warheads do is consecutive blast waves, which will destroy more buildings and cause more death due to collapsing structures.
High yield warheads are mostly for missile fields and bunkers, as a ground burst would send shockwaves in order to cause the missiles to be damaged or in the case of command bunkers overload their shock absorbers and cause collapse.

This issue with tsar bomba is no sane human can launch it. Nobody, no matter how tough, could withstand the guilt of killing millions with the push of a button
