In old Rus' chronicles there are two forms - Kiev and Kyev, but never "Kyiv". Even in the so called "first constitution of Ukraine" by "Pylyp Orlyk", it's Kiev.
And same for "Lviv", in old Galician-Volhynian chronicle it's written as Lvov.
