The "guns" are literally miniature darts with frozen poison in them. You get shot, it feels like a mosquitoes bite, but then as the compound thaws it causes a heart attack.
If you know of a compound that can cause a stroke it's likely they've done it.
I also suspect "heart attack" is just the general term. Most poisons/toxins have multiple attack vectors so "heart attack" could just be the vernacular for "cardiovascular failure."
