Yep - I got /one/, didn't have to /sign/ anything iirc, and certainly didn't give my SSN, so, it should be reasonably fakeable. They may check the source, though - I got mine at CVS, they may be able to query CVS as to whether someone got a shot at that time + place.

Your best bet would be to schedule to get a vaccine and when the nurse pulls out the stringe ask for the insert. Once you read say I am ready and point to the trash and refuse to roll up your sleeve. Make the nurse dump it and fill out your card. If they say no just leave and go to another place and try there.
