She killed herself by jumping off a roof and was reborn into Haibane Renmei, which began airing about a week after the last Azu episode. Osaka became Rakka.

Chiyo became the bird in Haibane Renmei. Picrel is Chiyo's "father", Chiyo-chichi, who is God. He exists outside time and physical limitations. That is how he can appear not only in Osaka's dream but also in Sakaki's. In picrel scene he expresses Chiyo's desire to become the bird who can fly to Osaka/Rakka to aid her (Chiyo-chichi himself is not actually a cat; he says to Sakaki that in truth he isn't a cat). That is why he's able to transform Chiyo into an actual physical bird.
