One, B-Class was the biggest baddest able to freely cross a portal. So that's what the wish granting council or what the fuck ever it was had available.
Two, Toguro died pretty much immediately after He went beyond his particular stage of B-Class by going beyond his limit. Perhaps that was simply the limit to his body.
