The biggest problem is that they must follow the same tropes everytime or else they can't publish their shit. It's the same reason every single fucking character in every single anime has the exact same voice and voice tropes.
It's also why once a very slight modification on the genre catches on, everything else stops being written completely (so now it's exclusively isekais, especially isekais with game concepts and UIs).
That's a jap society problem (they're turbo-insects), not necessarily their writers.
Similar to the above, every other time they right about fantasy, in a western setting, there is always:
- volcano-based onsen (never thermae or the kind of special waters you find in europe)
- Nobility rankings don't mean anything
- Japanese-style bowing, not european-style, complete with dogezas and everything
- Katana folded steel a thousand times so strong, etc.
- Everyone fawns over the MC introducing jap cuisine which is magically an instant delicacy, including such nonsense as omurice
- MC constantly complains about not having access to coca cola and burgers
- Somehow kings sleep on wooden planks with straw on top, and ducal carriages are made of especially hard wood. This is plot-critical because the MC is about to invent """modern comfort""".
This follows from the fact that japs are over-socialized, and this is important to understand their writing style (it is based on 'not rocking the boat' rather than actually being good, and it's important to understand that even the audience will refuse to consider something that is too outside the norm).

The problem is how the industry works. Why is ID:Invaded, despite it's speculative settings is a good story? This is because the author of this manga has won literary prizes (not for LN, for regular novels). Meanwhile most fantasy that are adapted, especially isekai, are authored by hacks that cannot make it in the industry and resorted to writing in Narou/Syousetsu. The sad thing is that Nip masses gobbled this up which led the industry have to cater for their taste

You're overexposed to shitty, cashgrab isekai and underexposed to the seas of worthless cashgrab YA garbage that western genre writing is inundated with. Truth is that no one can write.
You don't get it. When it's a random fantasy world (protip: literally never fucking happens), that's fine. When the story takes its western setting seriously (pretty much all isekais, the characters always reflect on how it's not japan, it doesn't work like japan, it is missing japanese fairs, and he is going to introduce those: a very common episode when onsens aren't a thing is when MC invents onsens and becomes rich through it), it's another matter. For example, the characters will very often handshake where the japs would just high-bow to illustrate that it's not japanese, and then 5 seconds later they're deep-bowing at each other while asking "please save my daughter".
This is also true, but the exposure rate is also a function of market reaction. Japs flock to isekai massproduced crap the same way westoids flock to marvel unwatchable turds, but there aren't that many 'marvels' and the things that usually get exposure beside those 'marvels' (which come and go) tend to be higher quality than trash isekai. A big reason for that is that trash isekai are often 1-2 men projects, westerners have higher budgets and bigger teams and such.
That said, the highest achievements are almost always in the west.
