there are plenty of non-isekai wish-fulfilment fantasies
but the very concept of a попаданец as a trope implies some fundamental difference, which usually comes in the form of modern day knowledge/foresight (let us ignore how sensible it is), and even then powers that be may explicitly grant protag extra shit
wtf is a survival genre in anime? older isekai was just an extreme fish-out-of-water scenario with some heckin' relatable ordinary high school students
there is a "stuck in a game" flavour of isekai, but it still involves some powerups (knowledge of the game, possession of a character with powers)
