Because they're not trying to be realistic, they're trying to create visually interesting towns.
That doesn't even look visually interesting.
Nothing else in Isekai is original, why not copy-paste the city design.

All these old European cities are full of so many unique distinct features that shaped their culture. It's not even laziness that these Isekai cities suck, just an utter lack of imagination.
And I could go on and on, there's so many cities I didn't post.
Most of the irl photos look more interesting and make more sense because they aren't a circle in the middle of nowhere.
